Parental Consent (Juniors and Youth Academy Only)
I confirm that the above particulars are correct. I consent to my child/children taking part in the activities arranged by Highland Hockey Club.
In accordance with the Scottish Hockey Guidelines, shin guards are required at all times and mouth guards are recommended whist playing hockey.
As part of our insurance cover and competition entry, each player must be registered with Scottish Hockey. The details submitted are: name, address, school, DOB &email address. All other details are retained by the club. Please see our data policy on the website.
I accept full responsibility for my child being collected on time from the activity.
I also indemnify the organisers against any claim for damages in respect of any loss or injury sustained by my child during (or as a consequence of) her / his participating in these activities
